What Is Blepharoplasty?

Blepharoplasty is a cosmetic surgical procedure that rejuvenates the eye area by removing or repositioning excess skin, muscle, and fat. With age, many individuals develop sagging skin above the eyes, puffiness below them, or changes in the eyelid-cheek transition. These concerns can make you look tired, stressed, or older than you feel.

Performed with precision and an artistic eye, eyelid surgery corrects these changes while preserving the unique character of your face. The result is a brighter, smoother, and more youthful look—often with minimal downtime and discreet incisions.

Upper Eyelid Blepharoplasty

One of the earliest signs of aging is the appearance of heaviness or fullness in the upper eyelids. This may create a hooded or folded appearance that conveys fatigue or tension, even when you’re feeling well-rested. Many patients also report difficulty applying makeup or feeling as though their eyes have lost their natural expressiveness.

Upper blepharoplasty involves the gentle removal of excess skin and, in some cases, muscle or fat from the upper eyelid. The incision is placed within the natural crease of the eyelid, making it virtually invisible once healed. At Robertson Cosmetic Center, our approach respects the natural fullness of the upper eyelids—avoiding the overly hollow look that was common with older techniques. This procedure can be performed under local anesthesia or mild oral sedation, and is often completed in under an hour. It is also commonly paired with other procedures, such as a facelift, in which case it may be done under IV sedation. Most patients return to normal activities in less than a week.

Lower Eyelid Blepharoplasty

Under-eye bags, puffiness, and loose skin can significantly affect how rested and youthful you appear. These changes may be caused by fatty protrusion, skin laxity, pigmentation, or descent of the cheek. Lower eyelid blepharoplasty is a versatile procedure designed to address these concerns with precision.

Depending on your anatomy and needs, our surgeons may use one of two primary techniques:

  • Transconjunctival Approach: Ideal for patients in their 30s and 40s who have puffiness but good skin tone. This incision is made inside the eyelid, leaving no visible scar. Fat is gently removed or repositioned to smooth the contour between the eyelid and cheek.
  • Subciliary Approach: Used when there is excess skin, visible wrinkles, or more advanced signs of aging. A discreet incision just beneath the lash line allows the surgeon to remove or reposition fat, tighten muscle, and precisely trim redundant skin.

Some patients may also benefit from a small “skin pinch” under the lashes for mild irregularities, or advanced skin tightening using laser resurfacing or chemical peels to complement their surgical results.

Our plastic surgeons at Robertson Cosmetic Center also have experience treating festoons—swelling at the lower eyelid-cheek junction. In early cases, they can be corrected through modifications of the standard surgical approach. More advanced cases may require direct excision for optimal results.

PRP (platelet rich plasma) is your very own blood that is drawn in office and spun down to separate out the platelets and plasma. Dermal Fillers

Our bodies produce less collagen every year after our twentieth birthday. Collagen is responsible for providing our skin’s underlying structural support. Loss of collagen, along with sun damage and other factors, leaves our face with areas of volume loss. This volume loss can be seen in sunken cheeks, a downturned smile, creases and lines, and an overall older appearance. When a filler is injected beneath a crease, such as a smile line, the filler simply returns volume to the area, pushing the skin back upwards and erasing the crease.
